There is some debate over this stitch and the diagram and stitch I have used is the one most commonly shown in embroidery reference books as "Buttonhole" stitch.  However, Marion Scoular, has made it her mission to distinguish true buttonhole stitch from what is simply a variation on the blanket stitch.  You can read her article and see her stitch diagrams by clicking here.
